If this is your first appointment with me, I will send you information to create a login for my client portal. You will be able to return to the client portal to schedule any of your future appointments. 

When scheduling the initial intake consultation appointment you will be asked to complete an intake form online. The information is stored securely so your information will be kept confidential. You will also be asked to enter your credit card information. You will not be charged until the time of the appointment. Please call or email me if you have any questions.
